What a bowtie diagram is.
A bowtie diagram is a barrier-based risk model. It starts with one hazard and one top event, then connects threats to preventative barriers on the left and consequences to recovery barriers on the right.
It helps a team distinguish stopping a loss of control from limiting harm after it happens. A bowtie is qualitative: it supports a structured conversation but does not calculate risk probabilities.

Common questions
What is a bowtie diagram used for?
It is used to map how threats can cause a loss of control and which barriers prevent or reduce the resulting consequences.
What is the difference between a hazard and a top event?
A hazard is a source of potential harm. The top event is the point at which control of that hazard is lost.
What is a preventative barrier?
It is a control intended to stop a specific threat from causing the top event, such as inspection, training or an automatic shutdown.
Can a bowtie show barrier degradation?
Yes. An escalation factor can show what may weaken a barrier, and another control can be added to manage that factor.
